We hope you all are having a good summer and everyone is staying well! The following is information for parents/guardians of students who will need to have medication administered to them during the school day.
For students doing in-person learning only:
- Click on the link for the medication form. Complete and have signed by a parent/guardian and your student's doctor:
- Please use one form per medication that your student will need to take while at school.
- For the medication drop-off procedure this year, there is a sign-up available for you to choose a time to stop by the school to meet with Nurse Stephanie in the front foyer at your scheduled time:
For students with asthma:
Due to the current COVID-19 pandemic, our district will only allow for the administration and use of inhalers for students with asthma during this school year.
Spacers and masks for the inhalers are permitted, but no nebulizer/breathing treatments will be administered on campus this school year.
Please discuss with your doctor about writing an order to administer the same number of puffs of an inhaler that is equivalent to the amount of medication in a breathing treatment, should your student need that.
For students doing virtual learning only:
No medications or medication forms will need to be submitted to the school while doing virtual learning.
Please make sure your student's health conditions and vaccination records are up-to-date with the school nurse.
